Synopsis

Undergoing surgery is not an everyday occurrence for most patients. Although patients have given consent for the surgery, many still feel a sense of helplessness. There will be feelings of fear and also having to cope with pain and an unfamiliar environment. There are however numerous opportunities for healthcare to address some of the touch points such that the experience of the patient is improved. This improved care can also work to reduce adverse events, especially in the less cooperative patient. This workshop is a collaboration between Khoo Teck Puat Hospital Perioperative Services and Humanitude organisation. Humanitude has transformed the way patients, especially the elderly and cognitively impaired are cared for. The workshop will:

 
  • Explore the care needs of the perioperative patient
  • Discuss how to heighten care and trust between the patient and the care team
  • Provide specific case studies to contextualise the principles
  • Provide practical tips and hands-on practice on the techniques described

 

Who should participate:

All healthcare workers that are involved in perioperative care including nursing staff and surgical staff.

This workshop is aimed at specialists and trainees from all acute care specialities involved in the care of perioperative, trauma, and critically ill patients, including Anaesthesiology, Intensive Care Medicine, Acute Medical and Surgical subspecialties, and Emergency Medicine.

Synopsis

This is a full-day workshop. The morning session will cover point of care ultrasound (POCUS) including airway, vascular, respiratory, basic cardiac views, and basic abdominal ultrasound for resuscitation, crisis management, stabilisation, triage and monitoring in perioperative, trauma, and critically ill patients. The afternoon session will cover focussed cardiac ultrasound (FOCUS) including both focussed trans-thoracic (FTTE) and trans-oesophageal echocardiography (FTOE), for cardiac triage and rapid identification of moderate to severe structural or functional lesions, cardiac haemodynamic measurements, as well as perioperative cardiac screening and fluid and haemodynamic management in non-cardiac surgery. Reading materials will be circulated prior to the workshop, so as to minimise lecture time and maximise practical hands-on imaging using simulated patients and a TEE simulator during the workshop;

Intended Learning Outcomes

At the end of this workshop participants will be able to :

  • Acquire the key POCUS and FOCUS views in simulated patients and a TEE simulator
  • Recognise images of moderate to severe pulmonary, vascular, abdominal and cardiac pathology for the differentiation of respiratory failure and shock, and identification of significant cardiac lesions
  • Perform practical POCUS and FOCUS applications for endotracheal and double-lumen tube confirmation, lung aeration monitoring, intravascular and extravascular fluid status monitoring, and haemodynamic measurements
  • Describe how to use POCUS and FOCUS for positioning and detection of complications related to VV and VA ECMO and IABP cannulae
  • Integrate POCUS and FOCUS with clinical management during emergency care of perioperative, trauma, and critically ill patients
