Background: Home-based post-acute care provides home care to patients who are disabled but can potentially be rehabilitated. However, acute disability often induces adaptation or emotion regulation problems in patients or their caregivers, and these difficulties cannot be properly addressed while the patients are hospitalized. Therefore, a tool that clearly explains the complicated services and conditions associated with post-acute care is necessary to facilitate healthy communication and promote the use of health services.

Materials and Methods: The graphic rehabilitation instruction form was adopted in October 2020. Therapists are required to provide verbal instructions along with the form to explain how services are to be delivered, service termination conditions, detailed scope of services, fees, and contact information. Furthermore, they are also required to provide guidance to patients and their families in setting up goals for rehabilitation. The number of patients who received home-based post-acute care and the number of those terminating the service were collected up to January 2021 and then compared with the numbers collected from an equivalent period before the introduction of the form to determine the effectiveness of the form.

Results: Between October 2020 and January 2021, the form was issued to 51 individuals. Among them, 37 used home-based post-acute care, but 4 terminated the service before its conclusion, resulting in an attrition rate of 10.8%. In comparison, 36 individuals used home-based post-acute care in an equivalent period (October 2019 to January 2020) but 6 terminated the service, resulting in an attrition rate of 16.7%. Therefore, the period in which the graphic rehabilitation instruction form was used had a lower attrition rate.

Conclusion: The introduction of the graphic rehabilitation instruction form enhanced information accessibility. It eased the performance of rehabilitation instructions and enabled patients and their families to more clearly understand the complicated post-acute care and participate in goal setting despite them being in a mental condition that put them at a disadvantage. Consequently, the use of post-acute care attrition rate decreased.
