Purpose:

Taiwan is estimated to become a “super-aged country” in 2026. Although the population of old people in Siao-Gang District in Kaohsiung City has reached up to 13.7% in 2016, which is higher than the rest of areas in the country, the coverage of long-term care was less than 30%. Therefore, how to increase the public awareness and community accessibility to long-term care services in order to provide local, professional, adequate and multivariate healthcare has become an immediate health issue.

Methods:

A local community integrative service center (Class A Unit) was established in our hospital since 2018. The service items of long-term care 2.0 program were promoted by patient support groups at local activity centers between 2018 and 2019. For applicants within Class Measurement Scale (CMS) tier 2-8, case manager conducted home-based interview to establish individualized care plan, and then connected the candidate with local multidisciplinary long-term care network for home-based care.

Results:

Since the opening of the Center (July, 2018) until the end of 2019, total of 907 citizens have applied for long-term care services. The coverage rate was increased up to 67.99% with averagely 35 new cases monthly. In the 5-point satisfaction survey, the results indicated satisfied to extremely satisfied with the long-term care services. 93% of the used services were crossover category items, suggesting that multidisciplinary service is essential to long-term care candidates.

Conclusion:

Case manager of community integrative service center is the immediate window to provide long-term care services from countries to villages. With increased service spots and extensive coverage areas, the accessibility to long-term care services becomes easier. The essential spirit and overall service quality of long-term care 2.0 program can be consequently carried out and improved accordingly.
