Abstract Content (abstracts should be written in Size 11 font, Arial font style) Population aging changes in disease patterns and extended life expectancy now is common in Taiwan. As the elderly accounted for 15.05% of the total population, Taiwan has officially become an aged society since 2018. It presumably will become a super-aged country in 2026.How to provide adequate multidisciplinary health care has become an immediate issue. A local community integrative long-term care service center (Class A Unit) was established in July 2018 by our hospital following national policy. A multidisciplinary team linked to local long-term care resource network along with exclusive case management is provided to evaluate the needs of the public for them to access to integrative services. Methods: According to the Class Measurement Scale by the Ministry of Health and Welfare, disabled subjects within CMS 2-8 tiers were the primary population for multidisciplinary long-term care services. The healthcare measures included: 1. Individualized care plan based on the actual needs of the case and the caregiver via on-site interview; 2. Individualized long-term care resource referral; 3. Service quality and efficacy assurance based on service satisfaction surveys, monthly phone calls and regular on-site follow-up; and 4. Expanded long-term service capacity among communities through the proper announcement. Results: After cooperation between interdisciplinary teamwork intervention and case management implementation, the results indicated that among 526 long-term care cases in 2019, monthly new enrollment was approximately 35 cases. The usage rate of long-term care services was increased, including home-based care (76%), transportation assistance (37%), respite services (55%), assistive devices (37%), and accessible environment (14%). In addition, total of 34 dementia referrals, 174 home-based rehabilitation cases, 14 home-based medical care services, 5home-based hospice care services, and 22 chewing and swallowing training services completed in 2019, resulting in an averagely 81% of the satisfactory rate in terms of all long-term care services. Conclusion: The combination of case management coordination, well-connected community services (between hospitals, rehabilitation facilities, social welfare resources and long-term care services), and home-based cooperation between interdisciplinary teamwork provided by a local community integrative center significantly facilitate the accessibility, visibility, availability and popularity of long-term care resources.
