Background

  Transplanters who are often discharged are often called to ask questions about home self-care after transplantation and the number of repeated inquiries is high.Once incomplete nursing guidance occurs, it is closely related to the continuous self-care of the transplanter after discharge from the hospital and the follow-up problems of complications. Back in 2018, the unit has begun to integrate resources, medical transplant team to cooperate with the whole process of care, formulate nursing guidance standard operations, establish color nursing instruction manual, and include unit case management telephone interview tracking and other measures to effectively improve the integrity of nursing guidance after unit kidney transplantation . Then, it was found that the completeness of nursing guidance after kidney transplantation increased from 36.6% to 98%

Methods

  This project is a series of improvement measures in the ward, such as: formulating kidney transplant care standards, kidney transplant care color health textbooks, dedicated pharmacists and dietitians, clinical care checklists, making patient kidney transplant question and answer test papers and other solutions.

Outcome

 After the implementation of this major, the completeness of nursing guidance after unit kidney transplantation has been increased from 36.6% to 98%, and the nursing guidance rate related to the clinical examination of kidney transplantation care items can reach 100%. This project effectively enhances the coordination and cooperation among the teams. The transplanters who have been discharged from the hospital call to inquire about the relevant home self-care issues after transplantation, as a reference for facing problems and clinical care.
