Introduction: Cancer patients often experience poor quality of life during chemotherapy treatments due to side effects including fatigue, nausea, vomiting, oral mucositis and neutropenia.However, the side effects are not alleviated, which will affect patient adherence to treatment and reduce the effectiveness of treatment.Receiving information about treatment-related side effects is a high priority for patients receiving chemotherapy.

Objectives: This study aimed to establishes the health education protocol, in addition to being used as nursing staff to follow, but also to enhance cancer patients' health literacy and self-care ability for chemotherapy.

Methods: Starting in March 2020, collaborated by the head of the nursing department and the ward nurse. Using the chemotherapy literature based on evidence-base medicine, to create a chemotherapy care guide protocol, based on side effects symptoms, self-care content-based structural forms, combined with the chemical treatment of health instruction manual, as a clinical nurse in the chemical treatment before, during and after nursing guidance tools, to guide patients and family members to understand side effects and self-care methods.After the form is established, the head nurse shall educate the caregivers on the instruction of use with structural forms, and after the implementation is carried out, the head nurse observes the actual execution of the caregivers.

Result: After intervention, not only the caregivers perform compliance of up to 100%, but also the structural forms and implementation progress included in the caregivers handoff.

Conclusion:

The results of observation after the use of structural form, the patients undergoing chemotherapy and family members not only increased the health literacy of side effects, understanding how to care, but also reduce concerns about side effects.After the use of structural forms, not only improve the relationship between medical staff and patients, but also to close the relationship between clinical caregivers and improve the continuity of care.This model could follow in the future, incorporating the concept of health literacy into more areas of care.
